Dr. Michael Potnek, DNP, AAHIVS, AGPCNP-BC, an internal medicine provider, was born in Menomonee Falls, Wisconsin. He earned his nursing doctorate from Marquette University of Wisconsin-Milwaukee and trained in settings including the Department of Corrections, an urban American Indian health center, and a large urban hospital. He is also a faculty member at Marquette University Graduate School. Board-certified in adult and geriatric medicine and an HIV specialist certified by the American Association of HIV Medicine, Dr. Potnek practices general primary care and has additional experience and interests in geriatrics, diabetes, kidney and lung disease, hepatitis C, and weight loss management. Dr. Potnek is passionate about optimizing his patients' health and wellness through a collaborative partnership, viewing his role as both a resource and a coach. He helps patients identify their health goals and devises personalized plans to achieve them, empowering patients through health education. In his spare time, Dr. Potnek enjoys fishing, working on cars, playing video and tabletop games, reading, and spending time with his family. He is excited to be part of CHS and values working with a fantastic team of practitioners.

Caitlyn Socha, APNP, FNP-BC, AAHIVS is a board-certified family nurse practitioner, providing compassionate and comprehensive care to patients of all ages. Caitlyn also specializes in management of individuals living with HIV and HIV prevention. Caitlyn enjoys working in Family Practice as this allows her to care for individuals throughout their life span, promoting a welcoming and trusting relationship. Caitlyn believes in building a strong relationship with her patients and focuses on preventative care, education and evidence-based practice to support long-term health and wellness. Caitlyn strives to create an environment for her patients that is welcoming, supporting and allows patients to feel heard, respected and empowered in their healthcare decisions. "I grew up in Beloit and it has been a privilege to give back to my community by providing comprehensive, well rounded health care to all individuals. Together, with myself and the patient, we can work as a team to develop a health care plan that is individualized to meet each individual’s needs and expectations. My goal is to educate patients on how to achieve a healthy lifestyle and reduce the risk of developing chronic illnesses."
