To get the process started, please fill out and submit the child and adolescent intake form and request the ADHD screening tools. The tools are helpful to have completed by your child’s first appointment so a timely diagnosis and treatment decision can be made.
Group counseling is an excellent way to both understand the impact that a symptom (such as anxiety) may have on your overall wellbeing but additionally it helps in the development of positive coping skills as group members can learn from each other as well as from the therapist who is facilitating.
We offer intensive substance abuse counseling to patients who are struggling with their sobriety or who have noticed that their substance use has begun impacting their personal or professional lives. In addition to IOP, treatment includes individual counseling, the possibility of medication assisted treatment through collaboration with Beloit Comprehensive Treatment Center, and development of a personal relapse prevention plan. We are an abstinent based treatment program and will encourage ongoing recovery support upon completion of services.

Patients can start and stop episodes of care as needed, depending on symptom stability. It is important to talk with your provider about requesting stopping, and how to reengage for future services.
You are able to transition to another counselor. CHS protocol is that the request is staffed to review better fits and which providers have capacity. This meeting happens on Wednesdays and an appointment with a new provider can be made after.
No – this is an insurance issue where only one visit will be covered, and the patient would be responsible for the other visit – we do not want our patients to have to pay for what would be covered by insurance if visits were on different days
Absolutely. We do ask that the initial appointment (assessment, psych evaluation) be completed on site to ensure appropriate documentation and paperwork is completed. Subsequent visits can be done virtually.
Absolutely. Please call our main number to make that request. We do have some clinicians who have remote days so please ensure that if you are wanting to see your clinician face-to-face, that your appointment is being scheduled during their on-site days
This is determined by your therapist at your intake appointment. During the intake, the therapist will assess needs, gain an understanding of your history and determine an appropriate level of care and frequency based on their clinical recommendations.
